I ran down to the weld shop and got some 1/16 rod and it’s going much much better now. With the bigger rod I couldn’t help but to make the damn bead so big lol.

I find it occurs more due to wire brushes than grinding disks but I have seen it from using a contaminated file to clean up the weld after complete as well.
